I understand that there are risks associated with boarding my pet(s) without being vaccinated for the CANINE INFLUENZA vaccine. I hereby waive and release House with a Heart from any and all liability of any nature, for any problems or illnesses this may cause as a result of lack of recommended vaccinations. I also acknowledge that if my pet shows any signs of illness prior to boarding, they could be turned away.*
